We write business exclusively through independent insurance agents and brokers. If you're not a broker, contact your insurance agent or broker for more information about our products and services. Or call or e-mail us for the names of agents and brokers in your area.

If you are an agent or broker, you can complete and sign an application online or send it by fax, e-mail or mail (pdf). In most cases, this short form application is all you need to bind.

For most accounts, we do not require financials or lost runs in order to quote the account.

You can also access our online rating tool by clicking the Premium Indicator^* link. This tool is provided for indications only. By entering the appropriate information, you can indicate the D&O premium for a claim-free association.

If the account has a history of claims, we may need additional information including:

• A completed and signed long form application
• Currently valued hard copy loss runs for the current year and four years prior
